Wash your hands often with soap and water for at least 20 seconds: ​
Lávese las manos frecuentemente con agua y jabón por al menos 20 segundos

Avoid touching your eyes, nose, and mouth:
Evite tocarse los ojos, la nariz o la boca
wear a cloth face covering over your nose and mouth: 
use una mascarilla  de tela para la cara sobre su nariz y boca

Facemask: mascarilla 
(others have translated this to "cubierta" but I have yet to hear someone use this term in practice)

Cover your cough or sneeze with a tissue, then throw the tissue in the trash: 
Cúbrase con un pañuelo desechable al toser o estornudar y luego bótelo en la basura

Cough: tos
Sneeze: estornudar
Clean and disinfect frequently touched objects and surfaces
Limpie y desinfecte los objetos y superficies de contacto frecuente
​Avoid contact with people who are sick
Evite el contacto cercano con persons que estan enfermas
